INITIAL APPOINTMENTS / FEES / CANCELLATIONS

Potential clients must complete the health history forms (see below) and mail/fax/drop off at the clinic in advance of an initial appointment.  For children under 10, please fill out the weekly symptom checklist form as well as a health history form. 

Carolyn is working roughly 3 days/week now as her husband has retired.  Although your appointment may be several weeks away, often new patients can be worked in sooner as cancellations arise.

FEES

Please Read Carefully:

An initial clinic visit with the nurse practitioner is $350 for a scheduled 90-minute consult (plus $100 per each subsequent ½ hour)

Please be aware that the first TWO clinic visits can average about 1 ½ - 2 hour each ($350 - $400 minimum based on 90+ minutes). Please be prepared/set aside this amount of time. It is not uncommon to be seen 3 - 4 times the first year to achieve balance. The amount of clinic visit time varies depending on a person's health care needs. Cancellation Policy:

Due to the tight schedule Carolyn manages, please know that you MUST contact P&H 24 hours in advance or will be billed $150 for not notifying us. Please know that there are other people who would love to have your appointment so please be kind and contact us in advance as requested. Thanks!

We can give you an insurance claim form to self-submit for those services that could be covered (Please read the Insurance and Financial Policy below).
